This form is for residents who haven’t recently received a letter from the NHS identifying them as being on the High Risk Shielded Patients Coronavirus (COVID-19) List. If you have received this letter please follow the guidance given to you in order to stay safe and contact the NHS on 0800 0288327 or at https://www.gov.uk/coronavirus-extremely-vulnerable for any assistance you need urgently.

If you are unwell, shielding or self-isolating as a result of Covid-19, and you need some support, please ask for help from others.

If you have a medical condition which makes you extremely vulnerable to coronavirus (COVID-19), register and tell the government whether or not you need support. Here’s some details that the government has published about their service.

You may have received a letter from the NHS telling you that you’re clinically extremely vulnerable, or been contacted by your GP or hospital clinician. If this has not happened, contact your GP or clinician after you register with this service.

Are you receiving Government food parcels which you no longer need?

If you received an NHS letter and have registered as shielding, you are entitled to receive a weekly food parcel. It contains enough food for an adult for 1 week. If you no longer want to receive these you can cancel deliveries if you wish.

To cancel food parcels there are 3 ways to do this.

  • In person
    • You can refuse the parcel on delivery.
  • Online
  • By phone
    • Call free on 0800 028 8327. Make sure you have your NHS number to hand. You’ll find this on the top of your NHS letter. This is an automated phone line and you will need to follow the instructions. Reply yes to ‘Do you have a way of getting essential supplies delivered at the moment?’. This will remove you from the government distribution list.

Warning Priority for supermarket delivery slots

If you cancel delivery of food parcels with your supermarket, you will still be on the shielding list and have priority for supermarket delivery slots.
